Disability Lifespan Solutions is a full-service practice dedicated to helping families promote the lifelong well-being of people with developmental, intellectual, psychiatric, and/or physical disabilities.

Disability Lifespan SolutionsEducational, financial, and treatment systems can be overwhelming to navigate. Disabilities Lifespan Solutions provides comprehensive, individualized guidance tailored to you and your family. Services include:

Lifecare Planning

Offer assistance developing and implementing comprehensive, personalized plans that address immediate concerns and future needs of families with people with disabilities. Ongoing assistance is available to monitor and modify plans as needs change over time.

“Turning 22” Transition Planning/Implementation

Provide comprehensive guidance and support managing the transition from child/special education services to adult benefits and programs. Beginning well in advance, we work to minimize stress and facilitate timely access to available benefits and programs.

Benefits Navigation

Guidance accessing and coordinating public and private benefits, including Supplemental Securing Income (SSI), Social Security Disability Insurance (SSDI), Medicaid, Medicare, SNAP (food stamps), and other cash and cash-value programs.

Identifying and Accessing Residential Programs

Help identify options for residential programs that meet individual needs and preferences as well as assist to secure desired placements and funding.

Healthcare Planning and Coordination

Assistance identifying and help coordinating communications with providers, including dentists and other specialists, who offer specialized services for people with disabilities.

Special Education Advocacy

Help parents and caregivers navigate special education processes to ensure that your child receives the special education services he or she needs.
